Fiscal Sponsor Acknowledgement Form

Should you be awarded a grant and have a fiscal sponsor, the Community Foundation will require a Fiscal Sponsorship Acknowledgement Form to be completed. This completed form will be required to be done prior to grant dollars being released to your fiscal sponsor.

In addition, a board resolution is required to be submitted alongside the Fiscal Sponsorship Acknowledgement Form. You can download the Board Resolution Form and have it signed.
